School Nurse/Health Services
Health and well-being provide the essential foundation for student learning. Our on-campus nurse strives to partner with parents, teachers, and when necessary, physicians to optimize student academic success.
Our nurse provides the following services:
- Tending to minor medical problems, such as headaches or queasiness
- Providing annual vision and hearing screenings for all pre-Kindergarten through first graders
- Providing vision and hearing checks every two years thereafter until 9th grade
- Conducting scoliosis screening in the middle-school years
- Managing health forms and records
- Overseeing medication distribution for students
Medication Policy
It is required that the school nurse have written doctor’s orders and written parental permission to administer any medication during the school day. This includes Tylenol, Advil, other over-the-counter medications, and all prescription medications. The school nurse has generic Tylenol and Advil on hand. Any other medication must be provided by the parent in its original packaging. It is the parent’s responsibility to safely deliver medication to the nurse. Parents must complete a medication policy form for each medication required. Completed forms can be dropped off with the school nurse or faxed directly to (513) 527-7638.
Emergency Medical Authorization Form
An Emergency Medical Authorization Form is required for every student. The form must be on file by the first day of school or the first athletic practice, whichever comes first. Students may not attend school or participate in athletics until this form is on file.
Lower and Elementary School Form
Completed forms can be dropped off with the school nurse or faxed directly to (513) 527-7638.
Student Medical Record Form
A medical record form is required each year for all students enrolled in any program below first grade. The form must be updated and signed by a physician annually.
This form is also required for all new students in grades 1 – 12 for purposes of documenting immunizations. It must be on file by the first day of school. Completed forms can be dropped off with the school nurse or faxed directly to (513) 527-7638.
Ohio High School Athletic Association Pre-Participation Physical Examination Form
A pre-participation form is required for all students in grades 7 – 12 who participate in athletics. A form must be on file in the athletic office by the first athletic practice.
