Form Submissions
All forms must be uploaded in Magnus. The school will not accept forms dropped off or faxed to school. Immunizations, physical exams, and birth certificates can be uploaded anytime to Magnus. See more information about Magnus in the navigation on the left side of the page.

ABOUT MACKENZIE SAFFIN
Mackenzie Saffin comes to Country Day with prior experience as a mental health specialist with CCHMC as well as several years as a nurse working in pediatric oncology, followed by time as a labor and delivery nurse. Mackenzie attended the University of Cincinnati to obtain her degrees in both nursing and psychology, and we believe both will serve her in taking care of our student population.
ABOUT JOANNA MANON
Joanna Manon comes to Country Day with several years of experience working in pediatric nursing. She has worked in the emergency department at Cincinnati Children’s Hospital since 2012 and began her nursing career on an ortho/trauma surgical unit at UC Health. She graduated with her BSN from the University of Cincinnati and also holds degrees in English and political science from Indiana University. Joanna is excited to bring her passion for health and wellness into the school setting, joining Nurse Saffin part time in our clinic.
