Magnus Health

1. It’s a digital, student health management system used by the school nurse and athletic trainers.
2. It collects and tracks forms so that we can keep your children safe and healthy.
3. When your child visits the clinic, treatment notes can be sent by the school nurse through Magnus.
4. Access through the Country Day app or on the Parents link on the top of the website.

You should have received a welcome email from Country Day that has a link and information. Please check the email for this information. Contact the Tech Help Desk with questions or if you need log-in help: veracross@countryday.net.

1. Log into Magnus as you normally would.
2. Hover over your name within Magnus (top left of screen)
3. Choose “Change Credentials”
4. Create a new username & password that only you know and will remember.  This new username/password will be used only with the Magnus mobile app or at MyMagnus.com.  Your previous Magnus credentials are only good through the Magnus Health Portal.
5. Download the “Magnus Mobile V2 App" from the Apple Store or the Google Play store and log in using your newly created username and password.

Once you have the Magnus mobile app downloaded to your phone, you’re ready to update information, sign forms electronically, and upload forms!

1. Open the app on your phone
2. Click on Student’s Name 
3. Choose Student Health Tracker and select the item you want to update.
   
   For example, if you have an immunization form already to upload, click on “Immunization Form”, “Submit Requirement”, “Add File” and then “Take a Photo.”  Be sure the photo does not cut off any of the form. 

If you have created new credentials for the Magnus mobile app, you must use the new credentials when logging into the Magnus app or go to MyMagnus.com to log into Magnus.  The Magnus Health Portal only works with your older credentials.

You’re done!

Please remember to have all RX forms signed by a physician and a parent.

1. A prescription medication form needs to be filled out if your child will be taking medication at school. A form is needed for Epi pens, inhalers, insulin, any medication at school or on a field trip.  This includes self-carry Epi pens and inhalers.
2. Log into Magnus, download the RX form, and complete it.  Bring the medication to the nurse’s office.
3. The OTC med form is NOT required, but necessary if you think your child will need Tylenol, ibuprofen, etc.  Any herbal or OTC med requires this form to be signed by a physician.

It is important to note that students who participate in fall sports need to make sure to get all requirements completed as soon as possible.   

Middle School:  All 7^th and 8^th grade students need the OSHAA form completed each year.

Upper School:  Only students on athletic teams need an OSHAA form.

Please do not wait until the last minute!

#1 Access via the Mobile App: (easiest method) or the Parent Portal on this website or link in your email on your computer or (any device)

#2 Begin uploading documents:
Here are the Step-by-Step instructions for you to do so on the mobile app:

• Select Complete Now beside your child's name 
• Scroll to the requirement that you need to complete
• Select Turn it In
• Choose the Upload option button
• Then snap a photo of your document

                              Or

• Submit required forms to the Magnus portal by scanning and uploading the documents on the computer .

You may also fax your forms directly to Magnus Health; this method requires that each individual form be accompanied by a unique, bar-coded cover letter. Log into the Magnus portal to obtain the letters and fax number. Processing times are much longer for this method so we strongly encourage you to use the mobile app or scan and upload documents.

The form is not signed by the parent and/or physician.
